If you decide to apply for the Partnership, follow the directions carefully so that all required information is submitted by 3 pm on April 16, 2004. JHU MMD Program Sequence of courses beginning June 1, 2004

APPLICATION to be completed by applicants and submitted with four items by April 16, 2004.

COMMITMENT LETTER to be signed and submitted with your application by April 16, 2004.

REFERENCE forms for applicants to send to two References, who submit by April 16, 2004. (you will need to print out both the cover note and reference form.)
Suggestion 1: Provide your references with a copy of the OVERVIEW of the Partnership.
Suggestion 2: Provide your references with a stamped, pre-addressed envelope.
