Establish Information System

Collect and utilize data to make decisions

    • Develop an office discipline referral, behavior incident recording sheet or tracking system that provides minimum information about
      • Who violated rule (name, grade)
      • Who observed and responded to the rule violation
      • When (day, time) the violation occurred
      • Where the rule violation occurred
      • Who else was involved in the problem situation
      • What was the possible motivation or purpose of the problem behavior
      • What school wide behavioral expectation was violated
    • Consider using School Wide Information System (SWIS) to manage data
    • Big 5
    • Complete and disseminate required reports/checklists on a specified basis

      • Form B: Ongoing Activity Monitoring
      • Form C: Coach’s Implementation
      • Form D: Coach’s Activity Log

    • Provide feedback to staff by ensuring that PBIS is always an agenda item during all-faculty staff meetings
